 note = {本研究の目的は,地域において援助困難とされやすい事例の看護学的構造を明らかにし,在宅生活を支援するための看護指針を得ることである。援助困難として訪問依頼を受けた事例への自己の看護過程のうち,在宅生活を支援する上で意味があると思われた7事例42場面を分析したところ,以下の結論を得た。1.援助困難として訪問依頼を受けた事例の看護学的構造地域において援助困難とされやすい事例は,家族員として「身体内部や認識内部に対立が起きている」「在宅生活を強く望んでいる」,家族として,心理的つながりの視点から「支え合って共同生活を営んでいる」あるいは「家族と対立し一人で生活している」,日常生活の視点から「自立が困難な日常生活を送っている」,地域の人々との関係において「家族と近隣との対立が起きている」「家族と関係者との対立が起きている」「関係者間の対立が起きている」という要因が重層的に複合して絡み合うことで,様々な看護上の問題が発生している,という構造をもつ。2.在宅生活を支援するための看護指針として,以下の7項目を得た(1)家族員それぞれの身体内部や認識内部の対立の有無を確認していく(2)家族員それぞれの在宅生活を望む意志や思いを探っていく(3)家族員同士の心理的つながりとその強さを探っていく(4)家族の日常生活の自立度を把握し家族の位置から選択的に社会資源を活用していく(5)近隣の人々が支える力となりうるかを探っていく(6)関係者が援助の対象としているのは,家族員個人か,家族員すべてかを把握していく(7)協同で関わる関係者の一員としての看護の専門職の立場にあることを意識していく
The purpose of this study is to determine nursing guidelines for difficult cases of home care. Forty-two illustrative scenes from seven cases which were recognized as difficult cases were analyzed. As a result, the factors in difficult cases were made clear. From the viewpoint of individual family members, there are "health problems (physical and/or emotional)", and/or "their insistence for on home care without regard to recommendations based on the health level of all persons in the environment." As for family relationships, "family members who have health problems were supporting each other" or "living alone due to bad relationships with other family members." With regard to the daily living ability, the patients are having "difficulty being independent." At the community level, there is "a conflict between families and their neighbors," and/or "a conflict between families and care providers" and/or "a conflict among care providers." These factors above were complicated. Seven items of assessment for the difficult case are recommended from this study: 1) physical and/or mental problems; 2) insistence for on home care of individual family members; 3) relationships and strength of family members; 4) degree of independence for daily living and utilization of the social resources depending on needs of family; 5) support from neighbors; 6) judgment of care providers to assess which family member needs a help and; 7) nursing professionals' awareness of their role among other health care professionals.},
