 note = {P(論文), 本研究の目的は,慢性疼痛をもつ患者が日常生活や社会生活を維持発展していけるよう,患者の自己調整力を支援する看護者に求められる能力を明らかにすることである。研究対象は,自己調整力を働かせ退院に至った3名の慢性疼痛患者と筆者との看護過程である。患者の自己調整力の働かせ方が変化していると思われる15場面をプロセスレコードに再構成し,研究素材とした。研究素材を分析し,「患者の自己調整力の働かせ方」がどう変化しているか,そのときの「看護者の判断」と「看護者の判断根拠」は何かを取り出した。次に,これらをもとに,「看護者の判断過程の特徴」を取り出して,さらに抽象化をすすめ,「看護者に求められる能力」を取り出した。全場面の「看護者に求められる能力」を内容の重なりから類別すると,以下の8項目になった。1)患者の抱えている対立が激化していないかという意識で患者の事実をみつめる 2)患者の事実から激化する対立を予測し,患者に確認することで患者の抱えている対立を浮き彫りにする 3)患者の言動を規定する認識を知ろうと関わり,患者がどんな願いや目標を描いて行動しているかを共有し,患者の自己調整力の働かせ方を浮き彫りにする 4)患者の自己調整力の働かせ方が健康の法則に沿っており対立を調和する方向に働いているか捉える 5)家族がどう患者の自己調整力を支えているか捉え,患者が家族の支えを実感できるよう関わる 6)患者の心の中の対立が激化して自己調整力が働かなくなっているときには,患者の苦痛を追体験してねぎらうことで患者の情緒を整え,新たな意志決定を引き出す 7)患者の自己調整力の働かせ方が高まるよう,調整手段を提示したり,調整を代行することで,患者の手段選択の幅を広げる 8)患者の調整を評価し効果を示すことで,患者の行動化や行動継続への意志決定を促す 取り出された8項目の能力は,看護者が,患者をどうみつめ,どう関われば発揮されるのか,看護の原基形態にそってまとめると,まず,患者の抱えている対立や自己調整力の働かせ方の個別性を浮き彫りにしようと患者の事実を相手の位置からみつめることが大前提となり,患者の自己調整力の働かせ方を健康の法則と照らし合わせながら患者にとっての意味を判断し,患者の自己調整力の高まりを敏感に感じ取って実感し,共有しつつ,意志決定を促し,支えるよう関わるとの結論を得た。, This study clarifies the ability of required nursing to support better self-management of chronic pain patients in order to improve their daily and social lives. Analysis of 15 nursing process records of three patients extracted changes of their self-management and basis of nurse's judgment which lead to the characteristics of nurse' s judgment processing. In conclusion, nurses are considered to be able to; 1) Observe patients' facts in order to determine worsening contradictions. 2) Predict contradictory conditions through patient communicational discovery 3) Understand and share patients' wishes and goals by defining their words and behaviors to assist in their self-management functions. 4) Observe how patient's self-management works along with principles of health and harmonizes contradictions. 5) Observe family support appropriately and help patient be aware of it. 6) Imagine pain of patients in heavy contradiction, sympathize and organize their feelings to lead them to new decisions. 7) Offer more possibilities by showing/helping patients achieve better self-management. 8) Evaluate patients' self-management and encourage further active patient decisions. The following are suggestions to display these ability for appropriate nursing for chronic pain patients; observe patients from their standpoint to better understand their individual contradictory conditions and self-management, visualize patients' present self-management abilities referring to the principles of health, understand and share the improvement of patients' self-management and encourage them to make their own decision for better self-management.},
